How to Get Started

If you are seeking counseling at the Redeemer Counseling Services, please follow the steps below. 

  1. Contact the Services Coordinator for a username to complete the intake forms online.  You may also request to have the paper forms mailed or e-mailed to you.

  1. Completed forms can be submitted online, mailed, faxed or dropped off at the address below.

  1. Upon receiving your forms, we will assign you a counselor and the Services Coordinator who will contact you within 24 to 48 hours to schedule your first appointment.  Afterwards, all counselors do their own scheduling.
Note: Each time you visit our main site at 1359 Broadway, you will need to sign in with photo ID at the front desk in the main lobby.  However, you will not need to sign in at our office on the 4th floor. Please wait for your counselor to meet you at the time of your session.


Couples and Pre-marital Counseling

Please contact the Services Coordinator for an online username to complete the intake forms online or to ask for a paper-copy of the intake forms.

Family Counseling

Each family member over 18 must submit a completed adult intake form online or in hard-copy. 
Please contact the Services Coordinator for the adult intake form at 212-370-0475 x1365 or rcs@redeemer.com.

For all children ages 3-12 and teens ages 13-17, parents or guardians must submit:

     Minor Intake Form (forms found below)
     Consent for the Treatment of Minors

For teens ages 13-17, parents or guardians must also submit:
    
     Adolescent Intake Form - To be completed by the adolescent (form found below)
